What’s the best approach to putting in a thin (0.5″) plywood subfloor over concrete slab?

I’m helping with a project that involves adding a 1/2″plywood subfloor over an existing slab in order to match the height of adjacent ceramic tile. They then want to install carpet tile over everything (over the plywood subfloor and slab, and over the existing ceramic tile). This is in an old bank building that’s being converted to a museum.

What’s the best approach for attaching the subfloor to the slab and blocking moisture with only 1/2″ to work with?

Would coating the concrete with a sealer like DryLoc, then glue/screwing the plywood overtop be acceptable? Would a poly vapor barrier be better?

Any other suggestions? Is there a better option than plywood to raise the floor level?

This is climate zone 4a, southern West Virginia.

    Probably easier and more permanent would be to use a levelling compound.

    Have you done a 3 day moisture test with a sheet of plastic and duct tape?

    Have you considered using a leveling compound and tapering out the .5 inch to 0 across 10-15 feet?
