Tight enclosure and gas appliance vents

How do you get a tight house if you have a back up gas burner for water heating?  There is the necessity for make up air supply plus the exhaust vent itself.   Plus any open bathroom fans.   Are these blower door tests done before the air vents and supplies are put in?
